Future of healthcare Essay

Future strategic direction plays a huge role in health care. In this paper, I plan to describe a minimum of five challenges that are defining the future strategic direction of health care. The challenges that will be addressed include the information technology advancements such as the electronic medical record/electronic health record challenge, the accreditation, quality of healthcare, and organizational compliance challenge, the access to health care including the uninsured and those in the poverty levels challenge, the market shares and advancing age of population challenge, and the maintaining a skilled workforce challenge. I will also describe how an organization can adapt its direction and strategies to effectively address these challenges. Technology, protocol, regulations, populations, and workforce skill will always change within time and how an organization chooses to address these challenges will determine its success or failure. The first challenge to be addressed is the challenge of information technology advancements such as the electronic medical record or electronic health record. To be used effectively, the EHR must have a meaningful use. There are several ways for the EHR to have a meaningful use. First, to have a meaningful use the organization must have a knowledge of the use and its benefits to the organization. There are several factors that must be in place for the meaningful use to benefit the organization. One such way is that technical resources must be available and  utilized effectively. Other ways would include vendor support, the use of information for specialists, the return on the investments, and the availability of state programs. FUTURE 3 Technical resources are crucial because there will always be problems that arise in the systems. Information for specialists must be available and easily accessed without invading privacy. There needs to be a return-on-investment to be effective. The availability of state programs has to be present to be effective. The challenges that may arise when adopting the HER process may include cost challenges. This would require knowing the cost of the infrastructure of the program, the training involved, and the technical support involved. This would require a great dealof budget planning to see a return on investments. Time would also present a challenge with this system because it requires more time on behalf of the employees that they normally would have been seeing patients. Preparation is a challenge because all stakeholders wo uld have to be in support of the adoption and all the work that goes into it. There has to be a well thought out implementation of the program and maintaining it to be successful. Another challenge could be the availability of vendors with the whole market on deadlines and with consultants and extension centers in high demand, availability could become an issue. Training is another issue that needs to be well planned and without adequate training, time, cost, customer satisfaction, and employee support from good training, there could be failure. Making sure that the new HER system operates fully with other existing systems is crucial and data migration from paper or digital resources could result in data losses. They are also very time-consuming. Along with the HER challenges, organization are also facing the challenges of accreditation and organizational compliance. There are strict rules and regulations as well as protocol to follow for an organization to earn accreditation status. Staff can provide to be a huge challenge in accreditation because many employees do not believe in the accreditation being an FUTURE 4 overall positive step to take because of all the necessary time, paperwork, certifications, etc. and skepticism about the credentials of the accreditors while reaching this status and maintaining it. Effectiveness  and cost eff ectiveness proves to be a challenge in organization’s looking to be accredited. It can become very expensive. Separating quality improvement from quality control can seem a difficult process. While accreditation processes basically guarantee quality improvement in an organization, just taking all the necessary steps and keeping employees on board can prove to be the biggest challenge. Another challenge would include the low number of accreditation boards available for hospitals. To face these challenges, having a good baseline accreditation process in crucial to the strategic planning. Education of managers with explanations of the advantages and benefits of the process will be needed. Using credible and knowledgeable evaluators and knowing monitoring standards can be proven key to success. An on-going data collection monitoring system can provide excellent ways of tracking in your strategic planning process. A project plan of core standards and time frames, mini-evaluations of requirements, and obtaining commitment from all staff will prove to be beneficial in the strategic planning process as well. Besides the accreditation and HER challenges, the future will also bring challenges in the access to healthcare by the under-insured and poverty level citizens. The issue here includes the overall healthiness of the under-insured and lower income people to have preven tative care and take care of pre-existing conditions. If these conditions go undetected, the outcome can become dire. These people are far less likely to seek treatment and will many times not report any problems in getting the necessary medical care. This is where the access to healthcare becomes an issue because if it is not reported, problems escalate with the available access. Many of these FUTURE 5 people do not seek care on a regular basis and do not have primary care physicians or other sources of care. With prices rising, the number of uninsured and under-insured has grown. If given prescriptions if they do get to seek care, many do not fill the medications because of cost and affordability issues. Many major illnesses are not caught on time and then they have to be hospitalized. After hospitalization, they are referred to a specialist but most of these patients do not follow up with the specialists again because of affordability. This leads to a huge lack in access to health care that is necessary. The Affordable Health Care Act and the expansion of Medicaid  services addresses many of these issues. Improvements in access, the utilization, and reduction of high out-of-pocket costs have reduced the number of people needing access tremendously. Strategic planning for the future would include safety nets of clinics and hospitals to care for the uninsur ed, charity care, reduction of costs, and prescription help programs. Federal funding and community communication to address needs and raise money with charities and donations will play an important role. The health reform has addressed many of these issues, but planning for the future, knowing what help is available, and leading these patients to the help available, will be necessary to maintain progress in this area. The challenge of market shares and advancing population ages is at an all-time high and expected to get higher. The U.S. population is aging. Home health with the population aging resulting in challenges in the delivery of healthcare, hospital consolidations becoming necessary, length of hospital stays needing to be reduced as well as growing Medicaid and Medicare customers are all examples of these issues. There is and continues to be huge growth in home health markets. The workforces are older and the patients are older than in the past with the future showing even more advancement in average ages. More retirement age workforces and FUTURE 6 patients that will only require home health or hospice services is something that is being seen more and more. The challenges to the workforce would include keeping a regular staff that will have many years of continuous work for the company so that we have available medical staff in our hospitals, medical office, clinics, etc. Strategic planning for the future would include strategies and recommendations on the recruitment and the retaining of skilled health professionals regardless of their ages. Identifying innovative practices to promote employment in healthcare even those with disabilities or past retirement ages can be aimed at retaining the experienced older staff and creating job opportunities for all ages. Maintaining a skilled workforce is yet another challenge that has been identified and needs to be addressed for the future of our health care. Research has shown that in workforce demographics and the trends of the workforce, hospitals will face low vacancy due to recession, shortages, and the tight labor market in the next decade. (Schidlow, 2008) Recruiting and retaining workers to replace  the retirees will be crucial. Strategic planning to include work process redesign, new technology implementation and maintenance, keeping employee satisfaction, and the attraction of a new generation of work staff will be necessary. ZEE: She’s a 19 year-old girl, suffering from anxiety and panic attacks. She is a quick-tempered, disorganized, untidy but clean girl. Although she tries to make others believe that she is a   liberal, in fact she is traditional. She has a strong sense of personal ethics. She can’t keep abreast of all innovations immediately. She suffers from insomnia at nights so she watches films when she can’t sleep. She’s addicted to cigarettes, but doesn’t drink alcohol. Most of the time she rings it off before the other person speaks. HALUK: He is Zee’s 78 year-old father. He lives in Turkey. He is a childish man. He still watches the cartoon Tom & Jerry every morning. He’s trying to keep up with technology in order to prove that he isn’t old. He’s not a narrow minded man; on the contrary, he’s innovative in the same way as young people and thinks in the same way with them. The fact that he   ZEE was born to him in his old age makes him care for her very much. Though he knows that his daughter loves him, he also wants affection from her. He calls her daughter at least ten times a day. HILARY ZANETICH: She is the closest friend of ZEE. She was born in New Jersey. She is blond, tall and plump. She doesn’t mind her physical appearance, except at nights when she goes out. She wears the same clothes at school everyday. She’s quite a flexible and a positive person. Although she believes that ZEE is a hypochondriac and crazy, she loves her very much. All the time she says that their friendship has improved her character in many ways. Her flexibility makes HILARY extremely funny. The fact that she never minds anything sometimes gets her into trouble. The first person that ZEE asks about America is HILARY. However, as Hilary’s general knowledge is not sufficient, she calls her mother each time after Hilary’s answers. YAMAN GAZIOGLU: He is the closest Turkish boy friend of ZEE. He is very keen on girls, especially American blond girls. His most noticeable feature is that while he is speaking English, he applies direct translations from Turkish. For that reason, he usually talks about what ZEE can understand. (He translates Turkish idioms into English, but they make sense to no one except for ZEE.) Turkey is against America. As images of Turkey come from the international map, ZEE explains about Turkish people and the general features of Turkey   that have been left behind. When the camera-shot moves away from Turkey and starts to approach America, we see luxury shops, automobiles, and elite people shopping and having dinner in Merrick Park, where Zee lives. Following these images, the camera is slowly directed to Zee’s bedroom. Meanwhile, we grasp from Zee’s words that she wants to be far away from her American life style even for only one day. Starting the day. The first place we meet Zee is her bedroom. Cigarette ends at the bedside of Zee, burying her head in her pillow, a great many dresses on the floor and DVDs draw our attention. A film is already on the TV with low volume. There are pictures of her father and his letters to her on the walls. Her father’s call wakes Zee up. Her father, whose computer has already been locked, calls Zee to ask for her help. However, he doesn’t listen to what ZEE says and does what he wants. Zee, fed up with her father’s advice and warnings, hangs the phone up. Meanwhile she is called again by Blockbuster so that she will return the DVDs she rented from them. As she can’t go on sleeping, she calls the hairdresser to have her hair done. But she can’t have an appointment since a customer has sued   the hairdresser. Extremely disappointed Zee deals with her hair by herself, gets dressed and goes out. Traffic monster. While she is driving slowly towards school   listening to music at the highest volume, she almost runs into an car trying to overtake a slower vehicle. She has to swerve to the right and runs into the garden of a house. She immediately looks at the number plate of the automobile and,   shaking   all over,   calls the police.   But the police cannot help Zee as neither she nor her car is injured. They say to her, that’s life. 4) Accusation Zee goes to her French lesson with a Turkish friend and her teacher makes them get out of the class. The teacher tells Zee that she will punish her and expel her from school as Zee helped a friend with his homework. Zee explains to her teacher that she’s still trying to get used to American school system, adding that in Turkey helping one’s friends with their homework is not considered such a bad thing. After that, the teacher excuses Zee by pointing out that she mustn’t do it again. 5) For those buying one, the second is free of charge. Zee goes shopping with her American friend Hilary in their one and a half hour break. The salesman tries to persuade her by saying that if she buys two more underclothes from Victoria Secret shopping center, a freezer will be given to her free of charge. The attitude of the salesman makes Zee angry. 6) Post- it When Zee comes back to school, she can’t find a parking place in the enclosed car-park of the school, so she has no chance out of parking on the meters. But she has no coins at that moment and she knows that her automobile will be towed away if she parks there as she hasn’t paid the fines she already has. She first hesitates but decides to park her car there as soon as she realizes that she’ll be late for her class. She gets out of   her car and   writes the following note on a post-it, putting it on the parking meter; â€Å"I’m sorry, but I have no coins. PLEASE DO NOT PULL MY AUTO AWAY, I WILL BE BACK WITHIN TWO HOURS.†Ã‚   She then sticks it on the windshield.

HPLC stands for High Performance Liquid Chromatography but is also referred to as High Pressure Liquid Chromatography. It is used for separating mixtures either to analyse the mixture or to separate a required product from others in a reaction mixture. It can also be used to find the relative amounts of different components in a mixture. HPLC works along the same lines as paper chromatography. In paper chromatography a liquid (mobile phase) moves past a solid (stationary phase). In paper chromatography the stationary phase consists of water molecules bound to the cellulose in the paper, the mobile phase carries different components of the mixture along with it. How fast each one moves depends on its relative affinity for the mobile and stationary phases. In HPLC the stationary phase is a solid packed into a column and the liquid is forced through the column by high pressure pumps. The pumps force the mixed solvents through the column the solvent emerging from the column carries the separated components of the mixture and is passed into the detector where a beam of ultraviolet light shines through it. There are many different types of detector depending on what you are analysing. This light is at a wavelength that is absorbed by all the components to be separated. When the detector reading drops the component that is absorbing the UV light is coming out of the column and passing through the detector. The time it takes for each component to come off the column is called its retention time and can be used to help identify it. For it is important to remember that Ralph Ranjit Kripal Sing, the exiled ex-politician hero of The Mimic Men is an insider, one who has practiced the most dubious forms of colonial mimicry as a politician and dandy, as husband and businessman and sees through the charade of politics, the deep humiliation and self contempt that results from defeat and failure. Naipauls The Mimic Men is divided into three parts and surprisingly the author has kept the name of the protagonist a secret until the beginning of the second part. The action of the first part takes place in London in the protagonist Ralph Sings youth. He has come there as a student on a scholarship. He lived in the boarding house owned by one Mr. Shylock. The house-keeper is a Maltese woman known as Lieni. The narrator counts plenty of experience to save himself from extinction and the resulting dust to dust; rags to rags; fear to fear ( Mimic Men 48). In fact, the word extinction has been used in a number of times in the novel. The situation where the narrator finds himself is an existential nothingness and he says, The tragedy of power like mine is that there is no way down. There can be only extinction (Mimic Men 48). By the time the narrator looks back and begins to write his past, he is forty. Later when the narrator took to writing like his creator Naipaul himself, he says, that he did so in order to give expression to the restlessness, the deep disorderwhich this great upheaval has brought about. ( Mimic Men 38) Part II of the novel talks about the protagonists unhappy childhood, unenterprising boyhood and the pressures that exerted him to leave his native island Isabella. He hailed from a poor family background in his words, On Isabella when I was a child it was a disgrace to be poor (Mimic Men 101). He descended from generation of idlers and failures. That caused him deep, silent shame (Mimic Men 101). Though his father was a schoolteacher and poor, his mother was from a rich family. Her brother Cecil was at school with the protagonist. He used to imagine that his father had landed on the island after his ship had been wrecked and he had lost all hopes of going back. Part II also talks about his Aryan background in India and how he added another name to his original name of Ranjit Kripal Sing. Ralph Sing is the example of thoroughly, psychologically colonized man, one who knows both the hurts and the excitements of the short-lived euphoria of inconsequential empires of our times. The burd en of guilt and betrayal, the consciousness of a collective shipwreck which all imitative and third-rate mimic societies suffer, makes Ralph Sing the voice of a quiet acceptance of colonial complicity. Ralph is the mimic man raised through a thorough-bred colonial education. The vision of disorder that haunts Sing is not only political, though. He can see no link between action and its result, man and landscape, the perennial reminders of slavery and brutalization. So, in the end a kind of neurosis sets in to carry Sing to the limits of self- derision. In part III the narrator Ralph Sing shares the ups and downs of his eventful life in Isabella soon after his coming over to that island nation with Sandra his wife from London. His marriage and his entry into politics were aberrations, whimsical, arbitrary acts(Mimic Men 219). Sandra quietly left him on a shopping trip to Miami. She never returned. And he never heard about her after that. It was with the instigation of one of his friends Browne that he became a politician. Browne was a man of the people. He was editing a newspaper called Socialist and was regularly contributing articles in it. Like Browne, Ralph Sing was not a thorough politician. He did not want to make a quick buck. The prospect of power in Isabella fatigued him. He felt he had no hold with the earth. He had no positive vision and hope. He had only a vision of a disorder which it was beyond any one man to put it right (Mimic Men 248). After 4 long years Ralph Sing was in a dilemma whether to return to Isabella as a failed politician or to stay back in London. In order to get some respite from his terrible loneliness and to boost up his self-image, he went to a brothel. With her also he failed as a sexual partner. His partner, to quote his words, was in despair. The smile of hysteria was replaced by tears; she reproached her self for my failure (Mimic Men 283). Quite secretly he went back to Isabella. He had thrown away his power. Such a state psychologically decolonized him. The mimic mans emancipation is impossibility, knowing the degree to which he has betrayed and violated himself, killing all truth and native purities. The precariousness of colonial or post-colonial leader, highlighted in The Mimic Men, is his slavery to the West on the one hand and his unease with his portion of the world. The imported culture, economy, industry, institutions and education are the mark of societies that Sing or Browne hoped to retrieve from disorder. Sings escape, his stay-back in London after the delegation has gone back to Isabella follows a penultimate, short visit to Isabella. But his ultimate exile in England is the most fruitful for him, for he is once again, as he says, in well-organised country and he has no wish to go back to the cycle of events he has freed himself from. Ralph Sing, the mover between cultures and geographies, politics and exile, finally turns a complete colonial, waiting perhaps to work on a history of the British Empire. Sings trying to find a settled role in a society defined by the extremes of Negro proletariat on the one hand and the ex-colonial Creole aristocracy on the other, presents the difficulty to place himself and the world he has come to. The attitude of the reader to Sings renouncing of public life, rejecting its emotions as fraudulent and settling down to write a memoir, will naturally be confused. Naipauls tendency for engraving and supplanting one narrative over the other reaches a limit in The Mimic Men. V.S. Naipauls postcolonial citizen helplessly reveals doubleness of identity in his existence whether in his island Isabella or in his city of choice and dream, London. Along with doubleness, irony and black humour also go with it. In fact such are the popular devices of Naipaul in all his fictional works. Ralph Sings increasing tension and alienation wherever he is, is a result of this awareness. He is also not able to identify with the colonizer though he find himself in his own colonizers state and gets the colonizers education and speaks the colonizers own language. The case of Ralph Sing it is simply not possible for him to shake off disposition and disunity. He chose London because he thought that a glamorous city like London could help him beat his sense of alienation. But London fails because Ralph Sing does not have a distinctive cultural identity with one particular culture. What Shashi Kamra points out as the general characteristics of a Naipaulian protagonist quite aptly fits in Ralph Sing ofThe Mimic Men: Naipauls narrator, subverting the chronological and objective order he has created through subjective ordering of his protagonists life and by questioning that order in his tone of irony and satire, creates the terror of placelessness and timelessness as a void- a pit without bottom. They should not be permitted because there are people with anger issues, violence , kills caused by alcohol, and committing suicide from depression/stress. Anger issues is one of many examples of why guns should not be permitted on college campuses. Having such an issue is already dangerous because people tend to take that anger out with yelling or fighting. People with that problem tend to snap quickly with just the wrong use of words. So theres no way of telling what might go through a persons mind having a gun on him. Would they take it out with shooting someone for a miss understanding? For example theres a class going on and everyone except one of the students is working, and the teacher tells one of the students to turn his phone in to her because he was on it instead of working. The teacher ask several times but the students decide to keep refusing to hand it to her. When the teacher decides to walk over and ask one last time he can easily take that as a threat because the teacher is in his personal space. When that goes on he decides to pull out his gun and shoots the teacher because of a problem so small. Not only is the teache r dead but the other people in the class are in danger as well. To prevent this from happening the option of weapons allowed shouldnt be allowed to the students. Another reason guns shouldnt be permitted is because of the use of alcohol. When being consumed with too much alcohol you dont think straight, which may cause you to make a bad decision, at the wrong time. There are many parties in colleges where students are introduced to huge amounts   of alcohol.   For example one of the most popular students on campus throws a huge party in one of the dorms where there is a lot of acholic beverages. After a couple of hours when everyone is drunk a guy gets a drink spilt on him. His reaction to the situation is going to be a lot different because of the fact that he is under the use of a high amount of alcohol. So when the drink is spilt on him he gets extremely mad so without thinking twice he pulls out a gun and shoots the person that spilt the drink on him. After this happens he starts freaking out because he realizes what he did so he gets scared and starts shooting all the witnesses there. From him carrying that gun caused him to put ever yones life at risk and because of him being highly influenced by the alcohol at the party. Theres already enough dangerous situations because of drunk driving at the campuses no need to put more dangerous situations. Another dangerous situation that can be caused is if there are people with violent behavior. Violent behavior can be defined as the use of physical force with the intent of physical force. An example would be if there was a class going on and one of the students in there is one of the violent students that dont like being told what to do. The teacher is giving the class because he was being loud and all the students were paying attention to him instead of the teacher. When the teacher decides to get on to him he tells him that no one tells him what to do that he doesnt care who it is and who ever disrespects him like that will pay the price. The teacher replys by saying that is his class and he talks to anyone however he wants. The arguing continues until the student finally gets tired of him he decides to reach in his bag and shoot the teacher. He just killed someone in front of a whole bunch of witnesses. So now he could either put his gun down and surrender or turn even more viole nt because there are witnesses around. Another option he could do is keep them hostages and keep them there until the authorities meet his commands. Everyones lives are at risk now because the opportunity was given to him to bring a lethal weapon. One of the biggest and most dangerous situations come from   depression and   stress. Most of the schools shootings that happened the year of 2015 were because of depression. There are plenty amount of shooting cases happen because of the amount of bulling going on in the schools from elementary schools to colleges. A great example is if theres a student being bullied throughout the whole school year just because he has no friends and always tries to fit in with the rest of the kids. He gets bullied because the way he dresses and because his parents arent financially   stable. Hes been holding all that anger of getting bullied ever since he was little because he knows in a fist fight he wont win against the other guys. In college he gets messed with too but even though it isnt as bad as it was in high school the other students still play pranks on him. Ever since the bullying started he would write down the names of the kids bulling just so he could remember who they were and the pain and embarrassment the would make him go through. One day in the middle of the year of his sophomore year of college he just had enough he was going through depression beca use he had no friends and he was going through a lot of stress because he was trying to keep his grades up but the other students would throw paper balls at him or just   laugh at his clothes theyd do anything to keep him from learning. The day he had enough though he started planning on what he was going to do to stop the problem. He would go to pawn shops and buy guns to help him go forward with his plan and since the college had permitted the students to carry concealed weapons,   he would have no problem getting in the college with the gun. After he buys his ammunition and guns now he just waits for the day of the plan to come and to finally get his revenge on all the students that made him go through depression and that would bully him into lots of stress. Next thing you know the day of the plan comes and he prepares everything he puts the small guns in his pockets and the bigger ones in his back pack. He pulls up to the college and goes to class like if it was any other regular day of school and as the class starts the students that always mess with him start throwing paper balls and spit balls at him. He gets up and turns to look at the group of guys. He reaches in his pocket and pulls the first weapon out and starts shooting one by one the students go down dead. Then he starts walking around the campus looking for the rest of the students on the list. After hes finished killing as many of the students as he can, he may commit suicide because he doesnt want to accept the consequences and hes juts tired of being bulli ed and stressed and just believes that its the only way out. The option of weapons allowed in   colleges shouldnt even be in mind. There are a lot of cases where there are people killed because of fire arm accidents and why put peoples lives even more at risk when theres no need to.
